World's Worst is a simple game featuring all four players standing on what became known as the "World's Worst Step". The players would then have to give examples of the world's worst something, often quite a humorous category. There was no pre-ordained order, and on some early British episodes, performers such as Ryan Stiles and Tony Slattery gave more than double the number of examples given by weaker performers.

By the time Whose Line appeared in the American format, World's Worst was generally relegated to a final game, often featuring Drew Carey getting in on the action. In fact, it had only appeared without Drew on one occasion - An episode where Greg Proops and Denny Siegel appeared together.

When the show was revived on the CW, the traditional step was no longer being used. Instead, the area used during Scenes From a Hat is used, with the camera shots being the same as the game Hats and Dating Profiles.
